Community Nursing Team

Providing a home vsiting nursing service for the housebound

First Contact Physiotherapist

The practice host a first contact physiotherapist for advice on musculoskeletal problems. An appointment can be booked via reception without the need to consult a doctor first.

Social Prescriber

John Keen-Tomblinson is the practice social prescriber. he offers help with the soical issues of care such as housing issues, signposting to support organisations, advice on local services. An appointment with him can be booked via reception.


The practice has access to 3 paramedics who work across the 5 practices that make up our PCN. The paramedics carry out both urgent and routine home visits which are arranged after consideration of the patients needs by their GP.


Poonam Jethwa is the practice pharmacist. She oversees medication reviews and monitoring and also carries out consultations with adult patients.

Physician Associate (PA)

Azna Edo is our PA. She carries out consultations in a similar way to the GP’s.
