REGISTER?

New patients who wish to register with the practice should collect a registration pack from the ground floor reception and complete the FP1 form immediately. An appointment will then be made with the health care assistant for a New Patient Medical. Failure to attend this appointment will result in a letter being sent, requesting that another appointment is made. If a subsequent appointment has not been arranged and attended within three months, the patient will be asked to register with another practice.
