DO I... To make an appointment either call into the surgery or telephone between the hours of 8.00am and 6.30pm. There is a duty doctor on-call every day to see patients who consider their ailment to be an emergency; the duty doctor will only deal with one problem and a routine appointment should be booked for anything that is not an emergency. New patients who wish to register with the practice should collect a registration pack from the ground floor reception and complete the FP1 form immediately. An appointment will then be made with the health care assistant for the New Patient Medical. Failure to attend this appointment will result in a letter being sent, requesting that another appointment is made. If a subsequent appointment has not been arranged and attended within three months, the patient will be asked to register with another practice. Home visits are at the discretion of the doctor. If you are genuinely too ill to attend the surgery, please telephone before 10.30am to enable the doctor to plan his/her rounds. Please give all the details to the receptionist as this will help to determine the priority. If you need to see a doctor immediately, tell our receptionist and you will be put through to a doctor. Please remember that a more complete and effective consultation can be done in the surgery with the benefits of our modern equipment and the help of our nursing staff. So always try to attend the surgery. Practice Registration Policy This surgery has an 'open' patient list and there is a registration policy for any patient wishing to register. The practice registers without any discrimination, but does retain the right to refuse registration on other grounds if it is felt justified. The reasons for which will be provided in writing in that case. Any patient wishing to register has to have a home address inside the practice’s catchment area (please see map on the wall at the reception desk or on this page). If you meet this first requirement, please then complete a GMS1 & New Health Patient Check form (ask at receptionist for a copy of both) and return them to reception after completion. Proof of the Following documentation will be required as part of the registration process Proof of ID = Passport, UK drivers licence or birth certificate Proof of address = Utility bill, bank statement, Tenancy agreement, benefits, Inland Revenue or official government letter (not hand written) that is not more than 3 months old. Proof of entitlement = Proof that the patient has been in the UK for 6 months, a work permit (which is not a holiday/work visa) or a student visa valid for over 6 months. Please note that a mobile telephone bill can not be used as proof of address. After registration, please make an appointment with the Health Care Assistant, to have a New Health Patient Check, at which you will be asked to provide a urine sample. |
